Transitions

Transitions is a 6 bed co-ed unit for adults that is designed for the specific needs of homeless people who have had extensive involvement with Community Patrol Services as determined by number of interventions or “pick ups” in the last 30 days.  Services are for up to 6 months with 5-6 months being recommended to address the very specific needs of this population.
