Clare Swan Outpatient Services
Outpatient services are provided at Nat’uh through the System of Care approach and are coordinated through Case Management. The Clare Swan Outpatient Services unit provides treatment services to adult men and women seeking Level I or II Treatment and Continuing Care.
About 40 patients are treated for substance abuse disorders but also receive referrals for mental health to treat individuals that require both services. All clients receive group therapy, life skills development, relapse prevention, sober support, family support, one on one substance abuse counseling, treatment planning and case management. Clients also can receive UAs on the unit which enhances accountability of sobriety.
Co Ed Adult Outpatient Unit is comprised of adult men and women who experience substance abuse or addiction and have been assessed and recommended to Level 1 Outpatient (OP) care requiring up to 8 hours of treatment which includes groups, individual, mental health counseling, case management and other activities aimed at encouraging sobriety and recovery. Clients remain in treatment for three to six months which includes Continuing Care.
Continuing Care Services provided for those adults who have just completed outpatient treatment. Continuing Care is individualized and can include a weekly group, one on one counseling and some case management prior to graduation. Continuing Care is a supportive time wherein the client maintains connections with the agency but generally is functioning independently with sober supports using the skills learned in treatment to remain in recovery.
